Policy recommendations for person-centred integrated health and care

AGE has drafted policy recommendations on integrated person-centred care as part of the EU-funded project SHAPES, which aims to better centralize digital solutions for personalized care.


The Horizon2020-funded SHAPES project aimed to build an interoperable platform integrating smart digital solutions to improve the quality of life of older adults with impairments and people with disabilities. The project adopted a multidisciplinary approach to conduct research on active and healthy ageing, identify people’s needs and provide personalised solutions that uphold the individuals’ data protection and trusts.

As a partner in this project, AGE delivered the “Pan-European Integrated Care Policy Guidelines”. These guidelines summarize the essential features of the concepts of integrated (person-centred) care and continuity of care and the key challenges and enablers of integrated person-centred care.

Geared towards tailored health and care services, adapting over time to the individuals’ needs, this paper is anchored in a human rights-based approach to promote health and autonomy across the lifespan. Our policy guidelines are intended to address European policy makers to work on governance, legislation, ethics and standards, finances, monitoring and reviewing, workforce, and research for integrated person-centred health and care.

The format of these guidelines – currently in the format of a project deliverable – will be adapted to be more user-friendly. They will be presented in the SHAPES event at the European Parliament on 5 September 2023 and on other occasions where European decision-makers gather.
